The ALJ, having reviewed the documents filed April 3, 2026 makes the following minute entry:

On April 3, 2026, the Department filed the attached documents with the Arizona

Office of Administrative Hearings (OAH). However, there was no indication that the documents were provided to Jeffrey Polley. The parties are reminded that any document filed with the OAH must be served on all parties. See Arizona Administrative Code R2-19-108(E).

The Administrative Law Judge did not receive the documents until after the hearing adjourned on April 3, 2026.

On October 20, 2025, William H. Doyle, Brandon D. Millam, and

Emily S. Morgan of Doyle Hernandez Millam entered their appearance on behalf of Jeffrey Polley. Emily S. Morgan, Esq. represented Mr. Polley at the hearing on March 16, 2026 and April 3, 2026.

Jeffrey Polley may file a response to the documents filed on April 3, 2026, by April 13, 2026.
