FINDINGS OF FACT
Mr. Halliburton holds national producer number license 21062710 issued by the Department. See Exhibit 1.
Mr. Halliburton’s address of record with the Department is as follows:

On February 14, 2025, Bankers Life and Casualty Company (Bankers
Life) submitted a complaint alleging that Mr. Haliburton’s contract had been terminated due to violations of his agent contract and agent compliance guidelines. See Exhibit 2.
Through its investigation, Bankers Life found that Mr. Haliburton signed
another person's name on a document and was paying fees or premiums for clients. See Exhibit 5.
On March 6, 2025, an investigator from the Department, Jennifer Cox
(Investigator Cox), emailed Bankers Life to acknowledge receipt of the complaint and to request documentation related to their investigation on March 27, 2025. See Exhibit 3.
Bankers Life sent its response to the Department confirming one
affected Arizona consumer, K.S. See Exhibit 4.
Bankers Life provided to the Department the applications for K.S.,
Bankers Life’s investigative report summary, and the termination letter that was sent to
Mr. Haliburton. See Exhibits 6 through 9.
In reviewing the K.S. applications, Investigator Cox confirmed
the following facts:
The four applications submitted by Mr. Halliburton for K.S. were submitted before Mr. Haliburton was licensed as an insurance producer in Arizona, on May 6, 2024, July 26, 2024, August 21, 2024, and September 26, 2024.
All four applications showed they were signed in Tampa, FL 33607, and the insurance email address was [email redacted].
The banking information included on the applications was for Tyndall Federal Credit Union (TFCU) and was verified to be Mr. Halliburton’s banking information.
Investigator Cox reviewed Bankers Life’s investigative report summary
and observed that Mr. Halliburton had participated in a sit-down interview with Bankers Life. The summary of the interview included the following:
Mr. Haliburton advised that he sold most of his policies via phone or zoom.
Mr. Halliburton electrically signed the insured applications himself after receiving permission from the insureds.
Mr. Haliburton advised that he was unaware that he could not write business outside the state of Florida and did not appear familiar with the state application form.
Mr. Haliburton indicated that he paid for the account policies from his business account which he had opened before being hired at Bankers Life.
Mr. Haliburton stated that clients would pay him and he would pay them for the premiums from his account at TFCU.

On March 27, 2025, Investigator Cox sent an e-mail to Mr. Halliburton
asking him to provide a narrative response to Bankers Life allegations. See Exhibit 10. The e-mail was returned as undeliverable. See the Hearing Audio Record at 40:00 to 45:00 minutes. Investigator Cox found another e-mail associated with Mr. Halliburton, [email redacted], and resent the email on March 28, 2025 requesting a narrative response by April 18, 2025. See id.
On April 23, 2025, the Department mailed a subpoena to
Mr. Haliburton’s address of record. See Exhibit 12. The subpoena requested a narrative response and relevant records related to the enrollments for K.S. The deadline to respond was May 14, 2025. Mr. Haliburton did not respond to the subpoena. See id.
CONCLUSIONS OF LAW
This matter lies within the Department’s jurisdiction.
The Notice of Hearing that the Department mailed to Mr. Haliburton at her address of record was reasonable, and Mr. Haliburton is deemed to have received notice of the hearing.
This matter is a disciplinary proceeding wherein the Department must prove by a preponderance of the evidence that Mr. Haliburton violated the State’s Insurance Laws.
The Department has established by a preponderance of the evidence that Mr. Haliburton sold insurance for a class or classes of insurance without a license for that line of authority, within the meaning of Ariz. Rev. Stat. § 20-282.
The Department has established by a preponderance of the evidence that
Mr. Haliburton used fraudulent, coercive, and dishonest practices, demonstrating incompetence, untrustworthiness, and financial irresponsibility in the conduct of business in this state or elsewhere, within the meaning of Ariz. Rev. Stat. § 20-295(A)(8).
Grounds exist for the Director of the Department to suspend, revoke, or refuse to renew the License pursuant to Ariz. Rev. Stat. § 20-295(A).
ORDER
Based upon the above, Mr. Haliburton’s License shall be revoked on the effective date of the Order entered in this matter.
Pursuant to A.R.S. § 41-1092.08(I), the licensee may accept the Administrative Law Judge Decision by advising the Office of Administrative Hearings in writing not more than ten (10) days after receiving the decision. If the licensee accepts the Administrative Law Judge Decision, the decision shall be certified as the final decision by the Office of Administrative Hearings.
In the event of certification of the Administrative Law Judge Decision by the Director of the Office of Administrative Hearings, the effective date of the Order will be forty (40) days from the date of that certification.
