In this case, a hearing was held and the administrative law judge issued a written decision. See Administrative Law Judge Decision, attached. MODIFICATION AND WRITTEN JUSTIFICATION Page 2, ¶ 5, of the Decision lists May 13, 2015, as the date Registrar issued the citation in this matter. However, the Registrar issued the citation on October 1, 2016. CONCLUSION AND ORDER

For sake of clarity, the Registrar hereby replaces “May 13, 2015,” found on page 2, ¶ 5, of the

Administrative Law Judge Decision, with “October 1, 2015.” The Registrar hereby accepts the remainder

of the Administrative Law Judge Decision, which becomes effective on April 20, 2016.
