The Administrative Law Judge Decision lists the status of License No. 147367 on page 4, ¶¶ 15 and

16. Respondent is not the holder of License No. 147367 and any reference to this license should be

removed from the Administrative Law Judge Decision.

CONCLUSION AND ORDER

The Registrar hereby removes ¶¶ 15 and 16 found on page 4 of the Administrative Law Judge

Decision. The Registrar hereby accepts the remainder of the Administrative Law Judge Decision, which

becomes effective on March 22, 2016.
