On page 3, line 25 ½ delete “not”, on page 4, line 25 delete “did not”. On page 6, line 10 ½ replace “CR21.0699226-D” with “CR21.069226-D” and insert “(14 days)” after “two weeks.” Justification: Correction of these facts is necessary to correct material issues in this matter and to ensure clarity of the decision. These corrections are based upon the factual record and hearing testimony. Based on this Decision, the Order, as modified, shall become effective on March 30, 2015.
