Based on this Decision, the Order, as written, shall become effective on June 26, 2013, and these matters shall be closed with prejudice. Any request for rehearing must be filed on or before June 21, 2013. Any appeal to the Superior Court must be filed on or before June 26, 2013. Note: Respondent failed to pay the $500.00 civil penalty assessed in the Registrar’s prior Order issued in Case No. 2011-5651. The Registrar received payment of the civil penalties assessed in the Registrar’s prior Orders in Case Nos. 2011-5655 and 2011-5656. Respondent shall have until May 27,
