The Registrar adopts the Administrative Law Judge's attached Recommended Order. Pursuant to the Registrar’s April 20, 2009 Order, License No. B-5.103029-R held by Respondent, shall be placed on disciplinary suspension for a period of five (5) days upon the effective date of this Order, followed immediately by a period of disciplinary probation of one hundred twenty (120) days. Based on this Decision, the Order, as written, shall become effective on February 19, 2014. IT IS SO ORDERED this 10th day of January, 2014.
