IN THE OFFICE OF ADMINISTRATIVE HEARINGS

On February 27, 2014, the Arizona State Board of Dental Examiners (“the Board”) filed the attached Request to Vacate Hearing a copy of an executed Voluntary Surrender agreement. Based upon the Voluntary Surrender agreement there is no need for a hearing in this matter. Accordingly, this matter is vacated from the docket of the Office of Administrative Hearings and remanded to the Arizona State Board of Dental Examiners for further proceedings.
